What is formation?
Formation is the preparation process parents attend prior to scheduling the baptism. Formation happens at home through three free videos and their study guide. These videos go over the theology of the sacrament and give parents an opportunity to dive deeper into what they ask of the Church and the baptismal promises they make. In addition to the at-home formation, parents attend two zoom meetings to go over the material, answer any questions that may come up during the at-home formation process, and help build community among parishioners who attend the digital gatherings. Twice a year we offer an in-person session for families that prefer to meet on campus.
Who needs to attend the preparation process?
Parents who have never gone through the preparation process for the Sacrament of Baptism for an infant (children who are 0-6 years of age) are asked to go through the process before scheduling the sacrament. To register for the preparation process, please fill out the form at the bottom of this page.
Parents who have gone through the preparation process for the Sacrament of Baptism, please check the section titled "Scheduling."
Godparents are not required to attend the preparation process. However, Godparents are highly encouraged to go through the formation and be as prepared as possible. If they wish to participate, they can register by filling out the form at the bottom of this page.

What do I need to provide to schedule the baptism?
There are four documents needed for parents to schedule the baptism:
1) Parent Registration form- this form is completed by the parent (enclosed below)
2) Godparent Registration form- this form is completed by the Godparent(s) (enclosed below)
3) Formation Verification Letter
4) Copy of child's birth certificate

Forms are sent to the Director of Liturgy, Joseph Reina. His contact information is also below.
Who can schedule a baptism at St. Edward?
Parishioners- we would only require the four documents mentioned above.
Non-parishioners- we would require the four documents mentioned above. Additionally, we will also need a letter from their pastor at their home parish granting permission for the sacrament to be celebrated at St. Edward.
All Godparents who are not parishioners of St. Edward will need to provide a letter from their pastor at their home parish stating they are members of the parish.
